Privacy Policy

Who we are

Telebiome operates telebiome.com, the public site for a vetted, compliant care network linking prescribers, pharmacies, in-person treatment centers, and brands. This policy explains what information this site collects, why we collect it, where it is stored, and the choices you have. It applies to this website only, not to the independent practices, pharmacies, centers, or brands that participate in the network.

What we collect

The only information this site collects is what you actively submit through a membership application form, plus minimal technical data needed to keep the site secure:

  • Contact details - your name, email address, and, if you provide it, a phone number.
  • Professional details - the fields specific to the membership you apply for, such as your credential and NPI (prescribers), your pharmacy name, type, and states served (pharmacies), your center name, type, location, and services (treatment centers), or your brand name, website, intended offering, and audience (brands).
  • Anything you write in the optional note field.
  • A one-way hash of your IP address - we store a SHA-256 hash (not the IP address itself) alongside your application, used only for rate limiting and abuse prevention.
  • Standard server logs - our hosting provider records routine request data (such as IP address and user agent) to operate and secure the infrastructure.

What we do not collect

  • This site has no user accounts and sets no tracking or advertising cookies. We do not run third-party analytics or ad trackers on this site.
  • This site is not a patient portal and does not collect patient health records. Membership applications are business and professional inquiries. Please do not submit patient information or any individual's medical details through the application form.

How we use your information

  • To review and evaluate your membership application.
  • To contact you about your application and, if accepted, your membership.
  • To verify professional credentials you provide (for example, checking an NPI or a state license against public registries) as part of vetting network members.
  • To prevent spam and abuse and to keep the site secure.

We do not sell your information, and we do not use it for third-party advertising.

Where your information lives

Applications are stored in a managed cloud database (Neon, a PostgreSQL provider). When you submit an application, a notification email containing the details you submitted is sent to Telebiome administrators through Resend, our email provider. The site itself is hosted on Vercel. Each of these providers processes data on our behalf to deliver the service and is not permitted to use it for their own purposes.

When we share information

We share information only with the service providers named above, when required by law or legal process, or to protect the rights, safety, or security of Telebiome, our members, or others. If your application is accepted, relevant details may be shared within the network as needed to onboard you as a member.

Retention

We keep application records while they are under consideration and, for accepted members, for the duration of the membership relationship. You can ask us to delete your application at any time using the contact below, and we will honor the request unless we are required to retain the record by law.

Your choices and rights

You may request access to, correction of, or deletion of the information you submitted through this site by emailing art.nuvo.art@gmail.com. Depending on where you live, you may have additional rights under local privacy law; we will honor valid requests consistent with those laws.

Children

This site is intended for licensed professionals and business operators. It is not directed at children, and we do not knowingly collect information from anyone under 18.

Changes to this policy

If we change this policy, we will post the updated version on this page with a new effective date. Material changes that affect how we handle already-submitted applications will be communicated to affected applicants by email where possible.
