Terms of Service

Agreement to these terms

These terms govern your use of telebiome.com (the "Site"), operated by Telebiome. By using the Site or submitting a membership application through it, you agree to these terms. If you do not agree, do not use the Site.

What the Site is

The Site describes the Telebiome network - a vetted, compliant network linking prescribers, pharmacies, in-person treatment centers, and brands - and accepts membership applications from professionals and businesses that want to join it. The Site is informational and an application intake. It is not a patient portal, a pharmacy, a telehealth service, or a marketplace, and no products or services are sold through it.

No medical advice

Nothing on the Site is medical advice, and using the Site does not create a clinician-patient relationship. Any care delivered within the Telebiome network is provided by independent licensed professionals under their own clinical judgment and their own agreements, not through this Site.

Membership applications

  • Submitting an application does not create a membership, partnership, or any other relationship. Membership is granted only through a separate written agreement after vetting.
  • You represent that the information you submit is accurate, that you are authorized to submit it on behalf of any organization you name, and that any credentials or licenses you claim are valid and in good standing.
  • You authorize us to verify what you submit, including checking credentials such as an NPI or state license against public registries.
  • We may accept or decline any application at our sole discretion and are not obligated to give a reason.
  • Do not submit patient information or any individual's medical details through the application form.

Acceptable use

You agree not to:

  • Submit false, misleading, or impersonating information.
  • Use the Site for any unlawful purpose.
  • Probe, disrupt, or circumvent the Site's security or rate limits, or use automated tools to scrape or flood the Site or its forms.
  • Submit personal data about others without authority to do so.

Intellectual property

The Site and its content, including text, design, and branding, belong to Telebiome or its licensors. You may view and share the Site for its intended purpose; you may not copy, modify, or reuse its content commercially without our written permission. Names of network members appear with their permission and remain their property.

Network members are independent

Prescribers, pharmacies, treatment centers, and brands in the network are independent entities responsible for their own services, licensure, and compliance. References to members on the Site are not an endorsement of any specific clinical outcome, and Telebiome is not responsible for services you obtain directly from a member outside a written agreement with us.

Disclaimers

The Site is provided "as is" and "as available." To the fullest extent permitted by law, Telebiome disclaims all warranties, express or implied, including merchantability, fitness for a particular purpose, and non-infringement. We do not warrant that the Site will be uninterrupted, error-free, or secure.

Limitation of liability

To the fullest extent permitted by law, Telebiome will not be liable for any indirect, incidental, special, consequential, or punitive damages, or any loss of profits, revenue, or data, arising from your use of the Site. Our total liability for any claim relating to the Site will not exceed one hundred US dollars ($100). Some jurisdictions do not allow certain limitations, so parts of this section may not apply to you.

Changes to the Site or these terms

We may change or discontinue the Site at any time. We may update these terms by posting a revised version on this page with a new effective date; continued use of the Site after a change means you accept the revised terms.

Governing law

These terms are governed by the laws of the United States and the state in which Telebiome is organized, without regard to conflict-of-law rules. Any dispute will be resolved in the courts of that state, and you consent to their jurisdiction.
